Community Health Promotion


ACHIEVE (Action Communities for Health, Innovation, and EnVironmental ChangE) is supported by the Centers for Disease Control and Prevention's Healthy Communities Program. ACHIEVE aims to bring together leaders from all sectors of the community to build healthier communities by promoting policy, systems and environmental change.

Areas of focus for ACHIEVE:
  • Physical activity
  • Nutrition
  • Tobacco cessation
  • Obesity
  • Diabetes
  • Cardiovascular disease

The ACHIEVE approach promotes health through such improvements as safe locations for physical activity, and increased acess to healthy food options at schools, worksites, places of worship and in the general community.

Bikers

ACHIEVE aims to combine the expertise of state and local government and public health departments with the experience of community based organizations that provide community support and outreach. PHDM will work to engage and mobilize leaders in our community to respond to our public health challenges of obesity, physical inactivity and poor nutrition.
