Age 2 Months 1st DTaP (Diphtheria, Tetanus, and Acellular Pertussis)
1st Polio (IPV)

2nd HEP B (Hepatitis B)
Age 4 Months 2nd DTaP
2nd Polio
2nd HIB (If Merck product, a series of 2 primary injections)
Age 6 Months 3rd DTaP
3rd Polio
3rd HIB (If Lederle or Connaught product, a series of 3 primary injections)
3rd HEP B (must be >=6 mos. of age)
Age 12-18 Months 4th DTaP
3rd or 4th HIB (Booster dose of all products)

Varicella (Chicken Pox - One Time Only)
Age 4-6 Years
(Before Starting Kindergarten)

MMR (May not be necessary if previously completed)

  • Childhood immunizations are available at little cost at Public Health.
  • Adult immunizations are available at little cost at Public Health.
  • Evening and Saturday appointments available during the school year.
  • Immunizations are also available through your private physician.
