At Birth	1st HEP B (Hepatitis B)
Age 2 Months	1st DTaP (Diphtheria, Tetanus, and Acellular Pertussis) 1st Polio (IPV) 1st HIB (Haemophilus influenza type b) 2nd HEP B (Hepatitis B)
Age 4 Months	2nd DTaP 2nd Polio 2nd HIB (If Merck product, a series of 2 primary injections)
Age 6 Months	3rd DTaP 3rd Polio 3rd HIB (If Lederle or Connaught product, a series of 3 primary injections) 3rd HEP B (must be >=6 mos. of age)
Age 12-18 Months	4th DTaP 3rd or 4th HIB (Booster dose of all products) 1st MMR 3rd HEP B (If not given at 6 mos.) Varicella (Chicken Pox - One Time Only)
Age 4-6 Years (Before Starting Kindergarten)	4th Polio 5th DTaP 2nd MMR Varicella (If not given prior.)

Adult immunizations are available at little cost at Public Health. Evening and Saturday appointments available during the school year. Immunizations are also available through your private physician. Minimum age requirements and intervals may effect schedule listed.
