ATTENTION Parent(s)/Guardian(s) of Children
Attending Immunization Requirements To enter a nursery, licensed child care center, preschool, Head Start Program, Pre-primary Impaired, or 4's at Risk Program: State law* prohibits a director from admitting a child to the program without a record of having received at least one dose of each vaccine: Measles, mumps, rubella, polio, diphtheria, tetanus, pertussis, Haemophilus influenza type b, hepatitis B, and effective January 1, 2000, varicella (chickenpox) vaccine. To stay in a nursery, licensed child care center, pre-school, Head Start Program, Pre-Primary Impaired, or 4's at Risk Program: You must provide the program director with a record showing that your child has received all of the following immunizations: Ages 15 MONTHS - 4 YEARS
CHILDREN who do not have the required immunizations WILL NOT BE PERMITTED to attend the program UNTIL the parent provides proof that ALL REQUIRED IMMUNIZATIONS have been RECEIVED. *Part 92, Act 368 of the Public Acts of 1978, as amended. **Children Ages 4-6 must have received 4 doses of pertussis. DT is only
acceptable if a signed waiver is on file for that particular dose of pertussis vaccine |
