- Information and assistance to access affordable healthcare services
- Assistance applying for free or discounted medications
- Drug card program for limited generic medications
Are you eligible?
- Resident of Delta and Menominee Counties
- Aged 19 to 64
- No health insurance
- Income below the following guidelines
- $20,800 for a single person
- $28,000 for a family of two
- $35,200 for a family of three
- $42,400 for a family of four
Who is not eligible?
- Those with any type of health insurance, private or public
- Those with work-related injuries involving worker's compensation
- Those with a pending court case involving medical expense or injuries
How to become enrolled:
- Call either our Delta or Menominee office to make an enrollment appointment
- Make sure you have with you:
- Proof of residency and age (Picture ID)
- Proof of income for the last 30 days (Check stubs or bank statements if auto deposited)
- List of medications you are currently taking, their dosages, and frequency at which they are taken
